Due to the recent issuance of numerous final and proposed regulations, we have once again updated our ever-popular HCR Provisions by Type and Size of Plan. The matrix lists HCR provisions and the years in which they became (or will become) effective, and notes if there are differences depending on plan or employer size, whether a plan is insured or self-insured, and whether a plan is grandfathered or non-grandfathered.
